Dnia 30 grudnia 1819 roku o godzinie 3 po południu. Przede mną księdzem proboszczem parafii Leszno, pełniącym obowiązki urzędnika stanu cywilnego gminy Leszno powiatu Błonie województwa mazowieckiego, stawili się: Franciszek Makowski rolnik lat 44 i Mateusz Dudkiewicz rolnik lat 38, obaj w Lesznie mieszkajacy, i oświadczyli, że dnia 29 grudnia tego roku o godzinie 5 wieczorem, we wsi Leszno w domu szpitalnym pod Nr 16, umarł Michał Kowalski, sługa kościelny z kościoła w Lesznie lat 70. Zostawił żonę Franciszkę nee Głąb, jednego syna Karola lat 25, córkę jedną Franciszkę lat 30.

Before me, the priest of Głusk and the official of the civil records office of the Głusk community, Błoński county, Warsaw department, appeared Piotr Koblak, according to his birth certificate from the Głusk church record book 24 years old, resident on his father's Piotr Koblak's farm, and Franciszka Goral*, according to her birth certificate from the same church 17 years old, residing at her family home with her father Jakub Goral*. Banns were read on 8 and 15th October. There are no impediments to the marriage. We announce that in the name of the law, Piotr Koblak and Franciszka Goral are joined together in marriage.
